About the Role
We are seeking a motivated and compassionate Intake & Outreach Specialist to join our growing team in Accra. In this role, you will manage a high-performing intake pipeline by converting referrals into scheduled patients through timely outreach, accurate assessments, and seamless coordination with clinical and scheduling teams. Key Responsibilities
- Manage and prioritize incoming referrals to ensure timely patient engagement and scheduling
- Conduct outreach to referred clients to complete intake screening and assessment processes
- Utilize standardized tools such as GAD-7, PHQ-9, and Columbia Suicide Severity Rating Scale to support clinical intake assessments
- Coordinate closely with clinical and scheduling teams to ensure smooth patient transitions into care
- Maintain accurate and up-to-date documentation of intake activities and client interactions
- Follow up consistently with referrals to minimize drop-off and improve conversion rates
- Communicate clearly and professionally with clients, caregivers, and referral sources
- Identify barriers to care and support clients in completing intake requirements
- Support continuous improvement of intake workflows and outreach strategies
- Uphold confidentiality and compliance with all behavioral health and data protection standards
Requirements
- Bachelor’s degree in Social Work, Psychology, Public Health, or a related field
- Minimum of 2 years of experience in behavioral health intake, outreach, or case management
- Familiarity with clinical assessment tools, including GAD-7, PHQ-9, and Columbia
- Strong communication, empathy, and interpersonal skills
- Ability to manage multiple cases and maintain attention to detail in a fast-paced environment
- Strong organizational and coordination skills
- Ability to work effectively with multidisciplinary teams
- Commitment to client-centered care and ethical practice
